Residential Medication Management Reviews (RMMR)

A Residential Medication Management Review (RMMR) involves an accredited pharmacist conducting a comprehensive assessment of a resident's medical history and medicines with the aim of identifying, resolving and preventing medication related incidents.

Permanent residents of an Australian Government Funded Aged Care Facility are eligible for an RMMR.

Under the Fifth Pharmacy Agreement, RMMR services must be requested by a resident’s General Practitioner (GP). Healthwise Medication Services conducts the RMMR and sends a written report to the GP as well as the Nursing Staff responsible for the care of the resident. The quality of the RMMR is a reflection of the pharmacists performing the review, and that is why Healthwise Medication Services insists on employing accredited pharmacists with significant experience in clinical pharmacy and all of which are geriatric certified.
